There are several ways an employer can report an injury to IWIF. Regardless of
the method of submission, IWIF will mail a hard copy of the First Report of
Injury (FROI) to the employer, the agent, the Workers' Compensation Commission
and the Department of Labor.
On-line Reporting
Click on Policyholder
E-Services to create a profile and register for
a PIN (personal identification number). If you already have
a PIN, click here to
enter your FROI on-line. Once the information
is submitted, you will receive immediate confirmation, a
claim number and
information for the ExpressScripts pharmacy program.
When an employee requires medical treatment and needs prescriptions
filled for a work-related injury, provide the employee with
a copy of the Temporary Prescription
Services ID Form (PDF) to take
to the pharmacy. When this completed form is presented to
the participating pharmacy, the prescriptions are filled with no out-of-pocket
expenses for the employee. Spanish

24-hr Injury Reporting Hotline
Call IWIF toll-free at 1-888-410-1400. A customer service representative will
take the information over the telephone and give you a claim number and
ExpressScripts number.
FAX Transmittal
Dial our fax number, (410) 494 2002, to submit your First Report of Injury. You
will receive confirmation via fax.
