Frequently Asked Questions about Injury Reporting
What is an "FROI"?
An FROI is IWIF's acronoym for an injury reporting form known as the employer's "First Report of
Injury." It requests information on such items as the date the injury or illness occurred, the
nature of the injury or illness, and they type of work being done.
How do I use IWIF's e-services, including the FROI?
In order to utilize IWIF's new e-services, you as a policyholder or agent must first create a
profile link and PIN number to gain access to these areas.
How do I get a PIN to use the electronic FROI?
To obtain a PIN, you must first create a profile link. To do this, simply click on the "Create
Profile" link (where is located?) and follow the prompts, entering the required information. As
part of this profile, you will be asked to create a user name. Your user name can be any
combination of letters or numbers containing five or more characters. You should make a note of
your user name, and it should be something that you can easily recall. Your PIN will be mailed to
the address listed on the IWIF policy through which the claim has been filed; it will be mailed by
U.S. mail in 7-10 working days.
How do I enter information on the electronic FROI?
After your user profile and PIN have been created and confirmed, you are ready to start using our
electronic FROI form. Generally, follow these steps:
- Once you have called up the form (be sure to enter the correct user name and PIN to access the FROI)
you can start entering information right on screen.
- Enter the required information and follow the prompts to complete the FROI.
- You will be able to review the input data as you complete the form.
- Be sure to add any pertinent comments in the appropriate field.
- After you electronically submit your form to IWIF, you will receive a claim number and
ExpressScripts number to signify completion of the reporting process.
- The information you submitted on the FROI will be forwarded to our Claims Department for review
and you will be notified in turn of the progress of your claim.
