The Maryland Workers’ Compensation Commission is asking employers to print their own copy. If you are unable to print a poster from the Maryland Workers’ Compensation Commission’s web site, the commission can supply you a copy upon request.

Effective October 1, 2007
The Maryland Workers' Compensation Employee Claim Form (MD WCC Form C-1) will be revised. The revised 3-page C-1 form will now include a mandatory Authorization for Release of Medical Information form with instructions relevant to the completion of the form.
